When is the questionnaire due?
Please complete the questionnaire as soon as possible. Time is of the essence during the hiring process, and delays may prevent the applicant from moving on to the next step.
Do I need to answer every question?
Some questions are required. However, if a question does not apply to you, you may type “N/A” into the field on the form.
Who will read my responses?
The background investigator, hiring manager, and command staff at the hiring agency will be the only people with access to your questionnaire. After a hiring decision is made, the background file is archived in a secure location. Your identity and responses are kept confidential and are not available to the applicant.
Will I have access to my completed questionnaire?
Unfortunately, no. Because of the confidential nature of the background investigation, no one other than approved staff at the hiring agency will have access to the questionnaires.
What if I want to change an answer after I submit the form, or provide additional detail?
Please call the Cooke & Associates Background Department at 925-856-9024. An investigator will be happy to speak to you to update a questionnaire.
What if my company has a policy that prohibits participation?
Please be advised that Government Code §1031.1 requires employers to disclose written employment information when a person is applying for a peace officer/public safety dispatcher position.
